i\u00a0regularnego dochodu\u00a0<\/strong>wystarczaj\u0105cego na\u00a0pokrycie koszt\u00f3w utrzymania siebie i\u00a0cz\u0142onk\u00f3w rodziny pozostaj\u0105cych na\u00a0jego utrzymaniu;<\/li><li>posiada\u00a0<strong>ubezpieczenie zdrowotne <\/strong>w\u00a0rozumieniu ustawy z\u00a0dnia 27\u00a0sierpnia 2004\u00a0r. o\u00a0\u015bwiadczeniach opieki zdrowotnej finansowanych ze\u00a0\u015brodk\u00f3w publicznych lub potwierdzenie pokrycia przez\u00a0ubezpieczyciela koszt\u00f3w leczenia na\u00a0terytorium Rzeczypospolitej Polskiej.<\/li><li>posiada
